Agreement & Acknowledgement

I am 18 years of age or older.

Photo ID must be provided. (We are required by the Illinois Animal Welfare Act to keep this information for our records as proof the animal was legally transferred/adopted into a home)

I understand that all members of my household must be in agreement with the adoption.

I understand that all dogs currently residing in my household may be required to meet the animal I am applying to adopt prior to adoption approval, unless otherwise approved in advance by SCAAC staff.

I agree to provide the adopted animal with appropriate veterinary care, including routine examinations, vaccinations, medications, and medical treatment as needed.

I agree to keep the adopted animal's rabies vaccination current in accordance with applicable state and local laws and ordinances.

I agree to provide appropriate year-round flea, tick, and heartworm prevention as recommended by a veterinarian.

I understand that submitting an adoption application does not guarantee approval to adopt a particular animal.

I understand that if multiple applications are submitted for the same animal, SCAAC will consider the needs and best interests of the animal when determining the most appropriate adoption match.

I understand that if my application is approved, I will be required to review and sign an Adoption Agreement before taking the animal home. I understand that the Adoption Contract contains additional terms and conditions governing the adoption and is a legally binding agreement between the adopter and SCAAC.

I understand that SCAAC reserves the right to deny an adoption application or decline an adoption when, in the organization's judgment, the adoption does not appear to be in the best interest of the animal.
